Enrollment Information and Forms
for All Day Care, Before & After School Care and Winter & Spring Camps

To register, it is REQUIRED that a complete enrollment packet be filled out for each child and submitted in person. For complete instructions, please follow steps 1-4 below.
|

|
|
STEP 1: Annual Enrollment Forms & Fee
|
|
|
Prior to attendance, parents are required to complete the enrollment forms for each child along with
payment of an annual non-refundable fee ($55/child or $80/family).
Annual forms and fee covers programming from June 1 - May 31.
Enrollment forms, enrollment fee and the first month’s program payment must be delivered at least 2 business days prior to your child's first day. Forms and payment may be turned in at the before and after school program site or at the front desks of the Lilley Gulch or Ridge Recreation Centers.
*Incomplete packets will not be processed and a child’s space in programs cannot be guaranteed.

STEP 2: Medication Needs
|
|
|
If you need us to administer any medications to your child, we must have a medication authorization form
completed by the parent and signed by a physician with prescriptive authority (i.e. Doctor, Certified Nurse Practitioner, or Physician Assistant, etc.).

STEP 3: Register for Individual Programs
|
|
|
In addition to the enrollment forms, a registration form for each program and/or camp must
be completed for each child.
|
|
STEP 4: Payments and Calendars
|
|
|
Calendar selection and payment must be submitted the month prior for each licensed program.
New families please contact your Site Director (see “Contacts” tab on the left) or
Administrative office at 303.409.2335 to obtain information on how to select your 1st month’s
schedule and pay tuition. Returning families can conduct their monthly business on the
Payments and Calendars page.

This paperwork is required and valid for all Licensed Foothills Children's Programs
All forms (Including Immunization Records) must be turned in 2 business days prior to your child’s first day.

CHANGE OF INFORMATION FORM
|
Change of Information Form - Parent/legal guardian or sponsor is responsible for informing Foothills Park & Recreation District
in writing of any changes in enrollment (i.e. change of address, phone numbers, etc.)
