SUMMER CAMPS 2012
Enrollment and Registration Information

STEP 1: Enroll in our programs - Early Enrollment Closed March 24
(Enrollments received after March 24 are assigned a Late Registration Date)
  To register for camps, all families must complete the annual enrollment process. The annual enrollment process requires that a complete, annual enrollment packet be filled out for each child and submitted in person along with payment of an annual non-refundable fee ($55/child or $80/family). Annual enrollment packet and fee covers programming from June 1 - May 31. *Incomplete packets will not be processed and a child’s space in programs cannot be guaranteed. Program Enrollment Forms

STEP 2: Register online for specific camps
(Takes place in April and May - see specific dates below)
  AFTER a completed annual enrollment packet and enrollment fee is received by the Children’s Programs Office, families can click here to Register Online for our camps. STEP 3: Pay monthly tuition
(Due the 1st of the month in June, July and August)

PLEASE NOTE:
The weekly options for Kids on the Move and Camp Hoppers are as follows:
 
  • 5 days a week - Monday through Friday
  • 4 days a week - Monday through Thursday only
  • 3 days a week - Monday/Wednesday/Friday only
  • 2 days a week - either Monday/Wednesday; Tuesday/Thursday;
        or Thursday/Friday only
  • We also offer a drop in option for one day/week - contingent upon space available

  • All Specialty Camps offer week long sessions only; drop-in is not available.

    APRIL 16:
    Early Online Registration begins (families currently enrolled in Before & After School, Preschool & ADCAP who completed Early or Open Enrollment by March 24.)

    LATE ENROLLMENT - MARCH 26:
    Enrollment packets received after March 24 are considered late enrollments. They will be accepted and assigned a late registration date.

  • Saturday - 12-4pm
  • Enrollment not available on Sunday

    April 23:
    Open Online Registration begins for ALL families who completed Open Enrollment by March 24.

    This paperwork is required and valid for all Licensed Foothills Children's Programs

    All forms (Including Immunization Records) must be turned in
    2 business days prior to your child’s first day.
  • Enrollment Packet* - Ages 7 and younger *Physician’s Report due to Children’s Programs Administrative Office (Ridge Recreation Center) 1 week prior to assigned Online Registration date.
  • Enrollment Packet - Ages 8 and older
  • Immunization Records - provided by your child's physician

    These forms are for children with special medical needs.
  • Asthma Care Plan (for Inhalers)
  • Allergy and Anaphylaxis Plan (for EpiPen or Antihistamine)
  • Administration/Permission of All Other Prescription or Over-the-Counter Medications (i.e. Ritalin, Tylenol, Cold Medicine, etc.)

    Change of Information Form - Parent/legal guardian or sponsor is responsible for informing Foothills Park & Recreation District
    in writing of any changes in enrollment (i.e. change of address, phone numbers, etc.)
